Hi Brad! When you switch to NR, will you remove the niacin from your supplement stack or will you continue to take niacin?
And regarding the niacin-induced blood sugar elevation, can metformin help with that?

I was having continued trouble with flush, even at 200mg of Niacin. Something that really seems to be helping me is “Extended Release Niacin” this is NOT “Slow Release” (slow release = less safe). While “Extended Release” is regular flushing niacin mixed into wax that dissolves over time giving you a small continuous release of niacin. Lets me do higher doses with little to no flush.
Great stuff, especially interesting because I am at that “metabolism falling off a cliff” age. I will start taking NR, Carnitine, NAC and L-Serine. Any dosage recommendations?
Hi Brad, thanks for the informative video. Would Niacin still be recommended for people with MTHFR variant and slow COMT? I ask this considering the possibility of Niacin increasing Homocysteine levels.

Thanks brad! Really fascinated by this topic. I’ve read that though Niacin raises blood sugar, it lowers hba1c. Are you aware of this? Perhaps it enables the blood cells to survive longer? Or high blood sugar is not a detriment when we have high nad to protect from oxidation and glycation? Perhaps always taking glycine or serine with niacin can balance this? Another though, I have gotten sever nausea from tmg. If the product gets oxidized its becomes TMA which is potentially toxic and beloved to be easily co better to TMAO. Any ideas in source or elements that could remedy this would be appreciated.
I am 55 and take 1 gram of niacin every day. i actually embrace the flush, as i think it has improved my circulation. it has eliminated the mild tingling i had in my feet. my hearing has improved. a small patch of spider veins on my ankle has completely disappeared, my blood pressure dropped from routinely above130 to below 120. and my hair has gotten thicker and longer. I had been taking NMN or NR for years before swapping out my morning dose with flushing niacin, so I attribute these things to the flush itself. (noting here that I also take TMG to help with the homocysteine levels.) for me, the flush feels like a good internal system scrub. though it does itch like crazy.
Thanks for the video. It seems you have changed your mind again about the Niacin dosing. Do you still plan to increase it from 100 mg/day to 1000 mg/day? Or will you keep just at 100 mg?
I’ve managed to create tolerance by increasing my intake slowly up to 500 mg a day in about 3 weeks.
Hi, yes after reviewing further data, I’ve decided not to increase niacin past 100mg. 500mg isn’t an extremely large dose compared to the dosages given previously for cholesterol control (was up to 4 grams)

Great info as always. QUESTION: How is metabolism measured? I’m 73 and would like to know where I stand regarding my metabolism level
I just started to take the Acetyl L Carnitine along with niacin which I’ve been taking for a while and I really think it may help with muscle recovery. My sciatica went away overenight it was quite persistent before that for months. Could just be co incidence but hasn’t come back and its been about 6 days now. So instant I wasn’t expecting that. Is this even possible? or is it placebo? Can’t be sure but I’ll keep taking its really cheap if you get in powder form like $30 for 250 doses but tastes bad I need some way to package it I might buy caps. If you have new data on this please share.
Any suggestions for timing the niacin? If it spikes blood sugar, is it a good idea to time it with exercise? Do you take it with a meal or on an empty stomach? Any issues with combining it with my morning (quarter dose 1/day) multivitamin which has a complete mix of B vitamins?
@Dr Brad Stanfield Wouldn’t taking niacin in small amounts frequently with each meal get around the high blood sugar issues because it’s inhibiting free fatty acids for a short time and never really allowing a free fatty acid rebound effect to take place after eating? Then during the night, we wouldn’t need to worry about a FFA rebound effect because we’re not eating.
I love the flush too, it wakes up my brain and my body, and as it comes on I hit it with caffeine, it is very synergistic with caffeine. On the other hand, any kind of sugar seems to ruin its effects
@Tree Walker good to know cause I don’t take sugar typically but it does lessen effects with food in general. On an empty stomach and water the flush is fast!

Acetyl-L-carnitine is different than L-carnitine and L-carnitine Tartrate. They work on different functions. Apparently L-carnitine Tartrate is the metabolic activator.
Very helpful info… helped me decide for NR instead of NMN, although quite the price difference! Niacin suggestion is superb! TMG a great addition in general in my opinion.

@Tony Mullins 🙂 The look like cherry part isn’t so bad, it’s the burn like a bonfire part of 500mg that I dislike.

Thank you, Dr. Stanfield for the ‘over 50’ recommendations. But when’s best to take all those amino acids? All together on an empty stomach? Bedtime? Before breakfast? Sprinkled throughout the day?
I have taken niacin daily for 4 years and I find it helps my energy levels. I don’t bother with NR or NMN as they are ridiculously expensive.
My problem with supplements is that if you get your nutrition from food, and I mean organic food, your body regulates what it needs automatically. If you add single supplements, and they are fat soluble I’m not sure that your body will be pleased with all those extra supplements. BTW, I take supplements (vit B-complex derived from yeast, vit D3 from sheep wool fat (only when there is no sun and during the winter, pure magnesium citrate (not every day) and zinc))). Other than that, I use sprouts from organic seeds like Broccoli sprouts, Alfalfa, Basil, Mustard seeds and so on. I eat green leafy vegetables, fruit, nuts, and seeds. I turn off as much electrical/electronics during the night so the EM fields cannot disturb my sleep. And I don’t use WiFi (unfortunately my neighbors do), but network cables. My pc is a few meters from my body. I think that these measures are much more important than supplementing your body. Oh, BTW, I also use cold showers (look up Wim Hoff), heart-brain coherence techniques and exercise regularly but not too much because exercising too much will reduce your immune system, I think. I try to fast sometimes but due to the meds I’m taking and I hate, it is nearly impossible to fast. I hope to get rid of these meds within the near future. Oh, I’m European and my first language is not English, so please bear with me if I wrote mistakes.
Dr, could you please talk about the possible insulin resistance that niacin can give? I have read this online and now I am hesitant about taking it or not. I am taking 100mg a day.
Hi Brad, I have been using NMN (500mg / day), Metformin (850mg), Niacin (500mg / 2 times per week), Quercetin (800mg / day) and my eyesight improved from -6.0 to -3.5 in Both eyes. I do not have diabetes. I am 65 years old. Is my improvement related to supplements? Is that normal? what’s going on? I do intermittent fasting daily from 8PM to 1PM the next day. I will be rechecking my blood glucose level to verify that there is no developing diabetes…
ahh and mine hearing that had deteriorated due to a surgery to remove a small tumor, I feel that it has improved… is that anecdotical o there is a cause – effect relation?
I take my aminos (EAA) in the am and pm on an empty stomach. I add Vit B6. Very helpful and energizing as a mid 50s woman.
@Matt Maze People that take Metformin live 10 years longer, on average, even diabetics.
You can replenish B12, you can’t replace your life.

Back in the day when I would catch the flu, I would use Niacin. The flush from the Niacin made the cold chills from the flu feel like I was on the beach getting a tan. Felt great!
@Super Mario but do you think people with low carb diet still need Metformin which make glucose level even lower? Maybe Metformin is not necessary stress for them.

Hi Brad, I was interested to take Niacin but I am currently being treated for premature coronary heart disease, and my cardiologist said I should not take a niacin supplement because it could interfere with my currently prescribed statin. As such, is there any other molecule that might have the same benefits as Niacin that you outlined but may not interfere with a prescribed statin? My cardiologist also said the Carnitine, Serine and NAC should be OK.
I used it with Covid. Really was remarkable. Like it was pumping life back into me. Gave me energy when I had none.
@tleich sure, it’s also the best treatment for long-covid right now, an auto immune condition caused by covid

@Dr Brad Stanfield how do you justify getting 1000mg metformin approved by pcp for longevity purposes if you don’t have high blood sugar & have it covered by insurance?!?
